Documents to submit after your baby is born

After childbirth, a mother's body is often as battered as if she'd been in a traffic accident, and with the baby who was inside her now in front of her, she often feels like "there's a person here" but hasn't had time to process her emotions.

This is an area where fathers can take initiative in parenting, so please do.

It's quite tough, even beyond just raising the baby.

As for the documents to submit, moms usually receive a brief explanation at the obstetrician's office or hospital, but dads don't get any explanation.

It's truly sad, so I've compiled this information to help you all prepare.

※The contents described here are just examples, so I hope you find them useful as a reference.

 

■Documents to submit

・Birth Registration (within 14 days of birth)

→What to bring: Maternal and Child Health Handbook (to get stamped), parents' seals, birth certificate

→Birth Registration Form: We received ours at the hospital.

→Where to submit: City hall or branch office

 

・Child Allowance 

→What to bring: Parents' seals, health insurance cards (for child and self), bank account number, My Number cards for all family members

→Documents: We received the forms and filled them out when we submitted the birth registration.

→Where to submit: City hall or branch office

※Child allowance details vary depending on circumstances such as dependents. 

 

・Health Insurance Card (not needed for 1-month checkup, won't make it in time)

→Documents: Copy of Maternal and Child Health Handbook or copy of birth certificate from birth registration

→Where to submit: For company employees, your company; for business owners, the pension office with jurisdiction over your residential area.

※This takes quite some time.

 

・Child Medical Expense Subsidy

→Process: Submit an application for the Child Medical Expense Subsidy to be approved, and receive the Child Medical Expense Subsidy Recipient Card.

→What's needed: Health insurance card required

→How to submit: Bring or mail to the Children and Family Division of the Health and Welfare Center in your ward.

 

・Birth Notification Form 

→How to submit: Electronic application is possible, submit within 7 days of birth

→Memo: Based on the birth notification form, a local midwife should contact you and visit your home for a consultation. 

 

・Childbirth and Childcare Lump-Sum Grant Application Form

→For us, it was applied at the time of payment at the hospital, so submission was not required.

Please check with the hospital where you are admitted.

 

・Child's My Number

→Notification of card completion: After submitting the birth registration, a notification card will arrive.

→What to bring: Baby's photo, recipient's My Number card, recipient's driver's license, etc., baby's health insurance card, baby's medical expense assistance certificate, etc.

※The necessary documents are listed in the envelope that arrives with the notification card.
